DISABILITY SUPPORT WORK
At Bass Coast Support Work Services, we provide personalised, relationship-based disability support that empowers people to live meaningful, independent and connected lives.
We understand that support isn’t just about tasks, it’s about trust, consistency and genuinely knowing the person.
Our team works alongside participants to build confidence, develop skills and create opportunities for inclusion within the community and beyond. We offer small group programs that cater to different personalities, energy levels and interests, because we know that one size never fits all.
Our Disability Support services include:
Personal care and daily living assistance
Community access and social participation
Transport to appointments and activities
Skill development (cooking, budgeting, independence skills)
Capacity building and confidence development
Support to attend education, employment or programs
Behaviour and emotional regulation support
Assistance with routines and structure
Support can be delivered in the participant’s home, in the community, at our Phoenix Rise property, or within one of our structured day programs.
We provide support:
1:1 individual supports
In-home supports
Community-based supports
We believe consistency matters. We aim to carefully match participants with support workers who align with their personality, interests and goals.
Our approach is:
Person-centred
Trauma-informed
Strengths-based
Recovery-oriented
Focused on dignity, choice and control
We walk alongside participants, not in front of them.
As a locally owned and operated provider in Bass Coast, we are deeply connected to our community. We support participants to access local events, programs, education, employment pathways and meaningful social opportunities.
We don’t believe in “filling time’’, we believe in building lives.
