Support Coordination Services (Victoria Wide)

We provide Level 2 Support Coordination across Victoria, supporting participants to understand, implement and maximise their NDIS plans with confidence and clarity.

Our Support Coordination service operates as a separate entity to Bass Coast Support Work Services (BCSWS). Coordination and direct support delivery are not linked or connected unless specifically requested by the participant. This ensures transparency, choice and control at all times.

Our Support Coordinators work alongside participants to:

  • Understand their NDIS plan and funding categories

  • Connect with appropriate service providers

  • Build capacity to manage supports independently

  • Navigate service agreements and provider options

  • Prepare for plan reviews

  • Troubleshoot challenges or service gaps

  • Strengthen informal and community supports

We focus on building long-term independence, not long-term dependency.

We work collaboratively with all service providers to ensure participants receive coordinated, consistent and high-quality support.
